Our Virtual Check-In Appointments make it easy to connect with your provider from the comfort of your home. Using secure audio and video technology, you can speak directly with your doctor or nurse practitioner through our patient portal, by telephone, secure email, or telehealth platform.

This option is designed for quick, convenient medical check-ins that:

Are not related to a medical visit within the last 7 days
Do not lead to a scheduled medical visit within the next 24 hours (or the soonest available appointment)
Have your consent on file for telehealth services

Please note: co-pays and deductibles apply in accordance with your insurance plan.

Our secure Patient Portal gives you convenient, 24/7 access to your healthcare information and communication with our team. Through the portal, you can:

Send a message to your provider and receive a response within one business day (excluding weekends and holidays)
Complete new patient forms before your appointment
View your medical records anytime
Access your lab results quickly and securely

You may call our office to book an appointment or to ask any medical questions with our Medical Assistant. If needed, one of our available providers will return your call within one business day (excluding weekends and holidays).

Your phone number
The reason(s) for your request

Telehealth

If you prefer two-way, real-time communication with your provider, we offer secure virtual visits through Doxy.me. Simply call our office to schedule, and one of our available providers will meet with you online within one business day (excluding weekends and holidays).
