This new patient paperwork must be completed by every new clinic patient. You can choose to:

1. Download, print, fill out paperwork and bring it to your appointment.

2. Use  Adobe Reader  to fill out form on your computer and either Email completed form to the office or print out the completed form and bring it to your appointment.

You will also need to provide a copy of your insurance card and state issued  ID card. You can take a picture of your card (front and back) and Email images to clinic or bring your card to your appointment, and we will scan a copy into your chart.
