At Sunstate Healthcare, we participate with a variety of insurance plans and make every effort to help you use your benefits efficiently.

Plans Not Accepted

We currently do not accept Humana or United Healthcare Commercial or Medicare plans, including Dual Complete, unless you have out-of-network benefits.

Out-of-Network & Self-Pay Options

We accept out-of-network insurance plans. If your plan isn’t covered, self-pay rates are: $200 for new patients | $95 for follow-ups. 

Authorizations

If you have an HMO plan that requires authorization from your Primary Care Provider, please try to obtain one before calling for an appointment.
However, our team will make every effort to assist you in obtaining authorization on your behalf.
