At Sunstate Healthcare, we participate with a variety of insurance plans and make every effort to help you use your benefits efficiently.
We currently do not accept Humana unless you have out-of-network benefits.
We accept out-of-network insurance plans. If your plan isn’t covered, self-pay rates are: $200 for new patients | $95 for follow-ups.
If you have an HMO plan that requires authorization from your Primary Care Provider, please try to obtain one before calling for an appointment.
However, our team will make every effort to assist you in obtaining authorization on your behalf.
You should always confirm your coverage directly with your insurance company, but our staff can assist you in verifying your benefits before scheduling your appointment.
