Because we care for many immunocompromised and high-risk patients, face masks are required for all visitors and patients while in the clinic. If you decline or are unable to wear a mask, we’ll ask you to cancel or reschedule your appointment until you can comply with this policy (we can help arrange a virtual check-in). We appreciate your cooperation in maintaining a safe environment for everyone. 

In accordance with insurance requirements and our office policy, all copayments are collected at check-in. If you choose to pay by credit card, a $3 processing fee (per transaction) will be added. This fee does not apply to cash, or checks. If you’re unable to provide your copay at arrival, we can reschedule your appointment for a later date.
